Health Care Navigator (SSVF Program)
Position Summary:
The Health Care Navigator connects Veterans and their families with essential medical and mental health services, helping them navigate the healthcare system and access the care and resources they need. Working closely with the SSVF Case Manager and mission team, the Navigator supports individuals in overcoming barriers to care and achieving better health outcomes.
Key Responsibilities:
Meet with clients to assess health care needs and goals.
Assist participants in finding, understanding, and accessing appropriate medical and mental health services.
Coordinate appointments, referrals, follow-ups, and necessary paperwork accurately and professionally.
Develop individualized health care plans and connect clients to community resources.
Educate clients about healthcare options, benefits, insurance navigation, and support services.
GPD Case Manager
Position Summary:
The GPD (Grant and Per Diem) Case Manager supports Veterans participating in the Mission’s GPD transitional housing program, helping them stabilize, increase self-sufficiency, and transition into permanent housing. The Case Manager serves as the primary point of contact for Veterans, coordinating services, developing personalized plans, and linking clients with community and VA resources.
Key Responsibilities:
Provide recovery-oriented case management and individualized support to Veterans in the GPD program.
Conduct intake assessments and develop personalized housing stability and service plans.
Coordinate referrals and follow-up for medical, mental health, legal, employment, benefits, and other supportive services.
Monitor client progress toward goals, adjust plans as needed, and advocate for client needs.
Facilitate crisis intervention and provide ongoing guidance through individual sessions and skills training.
Maintain complete and accurate case files and documentation in compliance with program and funding requirements.
Work collaboratively with community partners, VA liaisons, and Mission staff to enhance service delivery.
